Services

Care Plans: Our approach to care begins with the involvement of families and residents who are invited to become part of our team by attending the monthly resident care plan conferences held for each resident, and contacting our Resident Care Plan Coordinator with any approaches and interventions. Our Multidisciplinary Team includes our Resident Care Plan Coordinator, licensed Social Workers, our Dietician, members of our nursing staff, and Activities Coordinator from our Recreation Department.

Nursing Care: Our skilled nursing professionals are trained to make the initial evaluations for residents, initiate and revise nursing plans of care, and provide services for preventative and rehabilitative care. Our Nursing Department handles tracheotomy care and IV therapies, (including porta caths and TLC), Foley Catheter procedures, pain management, preservation of skin integrity, vascular and circulatory diseases, respiratory disease, blood sugars, peritoneal dialysis, and oxygen saturations, care for degenerative diseases, TBI and paralysis, including care for young adults. Nursing Aides assist with mobility training, self-care activities, swallowing disorders, communication disorders, orthotics and prosthetics.
Our nursing staff is able to assist residents and their families cope with illness, emergencies, and long-term disease, and will educate patients in healthy living and wellness. Medical staff coverage is provided seven days a week. Bridgeport Health Care Center has a nurse practitioner on staff with advanced education and training in the diagnosis and management of medical conditions, including chronic illnesses, and maintains close working relationships with physicians.

Alzheimer’s Care: Our facility offers a specialized nursing unit dedicated to the compassionate care of Dementia and Alzheimer’s patients. Our program provides care for a loved one facing the emotional and physical effects of Alzheimer’s and related dementia. That care is provided in a safe and supportive environment, and our patients are provided with daily, meaningful activities to keep them engaged.

Rehab/Physical Therapy: Our physical therapy services provide a valuable link between a short-stay individual’s hospitalization and that resident’s return home. Working with the resident’s physician and our nursing care staff, together with the skills of our physical, speech, and occupational therapists, maximizes the recovery potential of each resident.

Wound Care: Our Wound Team which includes our Infection Control RN, Wound Care RN, and Treatment LPN meet weekly with Administration to assess, audit and track and trend any wound issues, as well as to discuss preventative measures to ensure the comfort of our residents.

Dietary: A Registered Dietician is on staff to advise and assist with designing a meal plan for our resident’s, and our Dietary Department offers restaurant-style dining for lunch and dinner in our first floor dining area. Meals are prepared by Johnson-Wales chefs and trained cooks. (Please feel free to access our dining menus above under “Amenities”).

Recreation: Recreational activities such as coffee hour, arts and crafts, games, baking, flower arranging, morning socials and exercise class are offered daily, and include Catholic Mass, Protestant worship, spiritual readings and bible study, and musical entertainment is provided on weekends. The Recreation Department hosts our annual Senior Olympics in September and Nursing Home Week in May, as well as out trips to movies, baseball games, and area restaurants. Respite Care: At Bridgeport Health Care Center, we understand that caring for a loved one who is ill can be exhausting and emotionally draining. When caretakers need to rejuvenate or travel, we provide respite services. We will care for your loved one overnight or for an extended stay.

Social Services: We understand the importance of professional nursing care, but we also recognize the importance of good mental health. Our Social Services Department helps residents become accustomed to their new surroundings and daily routine, and offers assistance to residents during their initial admission. Daily intervention and problem support is also provided to our residents. Our Social Services Department is key in the discharge planning of our residents, and our social workers are diligent with regard to ensuring that all services are available to a resident who is transitioning back into the community.

Hospice Care: Bridgeport Health Care Center supports patients with terminal illnesses, as well as their families and caregivers, with the determination to preserve each patient’s dignity through comfort care, pain control and symptom management. Our goal is to elevate the quality of life for those in need of Hospice care.
